You are responsible for obtaining the correct directions to our location.
CANCELLATION POLICY
A “missed appointment” is any scheduled appointment you fail to show up for at the allotted time. Failure to comply will result in a $75.00 penalty, that you are personally responsible for paying.
A “cancellation” is defined when a patient does not give AT LEAST 24 hours’ notice of cancellation. Failure to comply will result in a $45.00 penalty that you are personally responsible for paying.
Remember that your appointment time was reserved for you in advance. You are responsible for remembering your appointment date and time. We require AT LEAST 48 hours’ notice if you need to move/cancel/reschedule an appointment. If it is a family/medical emergency and you are unable to maintain your appointment, we still ask for as much notice in advance as possible. If you are scheduled for a Monday appointment and need to move/change your appointment, please call the Friday prior. By providing the office notice, you are allowing us to fill that time for another patient in need.
Our office does courtesy reminder calls/or texts. This is done by an automated software. To receive appointment reminders, you MUST participate in our “Patient Portal”, and leave a valid telephone number. If you do not wish to participate in Patient Portal or do not have access to an email account, you will receive an appointment card for all upcoming appointments. You will not receive a reminder call. Not having a voicemail box set up or a full mailbox is not a valid excuse for any missed appointment, and you will be charged. Not receiving a reminder call/text is not a valid excuse for any missed appointment, and you will be charged.
If you are unable to maintain your previously scheduled appointment, call the office AT LEAST 48 hours in advance, in order to avoid penalties being added to your account. This is a charge that is not covered by insurance. It is critical for our patients to maintain appointments and follow through with their recommended care.
WAITING ROOM ETIQUETTE
Please be respectful!
Our waiting room is not a place for you to make phone calls. This time was reserved for you! Keeping your provider waiting for you to make a "quick call," will not be tolerated. It is disruptive of our administrative staff, as well as any other patients who may be waiting. Please limit phone calls to your personal time.
Please remain in the waiting room until your provider comes to get you.
You are permitted to have food and drink inside the office, please be courteous and clean up after yourself.
Check in with Office Administrator before leaving.
PRESCRIPTION REFILLS/QUESTIONS
Prescription refills MUST be called in to the office on the Prescription Line.
Any prescriptions left in a general mailbox, or email inbox, may not get addressed in a timely manor.
Any questions pertaining to your medication must be left on the Prescription Line.
All calls are addressed in a timely manor.
